What this form is for
Clarité Dubai uses your name and contact details to record your interest and respond to your enquiry. Optional professional, social account, general location and preference information helps us understand demand and plan our opening.
Your choices
Occupation, social account, email and preferences are optional. Providing a social account does not give permission to message it. Marketing consent is separate: we only send opening news, booking updates and invitations through the channels you select. You can register without opting into marketing.
Storage and access
Submissions are stored in the website’s hosted database and accessible through a restricted clinic administration area. Hosting service providers process this information to operate the website. We do not publish your responses. A temporary, one-way hash of your network address is stored to limit automated submissions.
Updating or removing your details
You can withdraw marketing consent by replying to a message. To request correction or deletion, contact the Clarité team through our official Instagram account. Please do not send medical documents through this form or social media. We retain registration information while managing the opening and related enquiries, and remove it when no longer needed or on an applicable deletion request.
A registration, not medical care
This form is for adults aged 18 or older registering their own interest. Do not include medical history, photographs, identification documents or a full home address. Registration is not a diagnosis, treatment recommendation or confirmed appointment.
